Battelle requires additional information for reporting, safety and compliance requirements. Below are forms Battelle may require subcontractors/suppliers to complete in addition to other documents issued with the purchase order or subcontract.

Subcontractor Property Survey: Required
when a subcontractor/supplier has or will obtain government furnished or acquired property.
Significant Financial Interest (SFI) Disclosure Form: This form is required to be completed by each Investigator pursuant to Battelle’s Policy on Financial Conflicts of Interest in Public Health Service (PHS) Funded Research.
Safety Supplement: Contractors and contractor personnel are responsible for maintaining a safe working environment and following safe work practices while performing work at Battelle.
Supplier Quality Survey: Key and significant contractors and suppliers will complete this form to assist Battelle in evaluating the subcontractor/supplier's quality systems and compliance with DoD Counterfeit Parts program.
